Report of Deceased Member

National Headquarters

Use this form to report the death of a chapter member to National Headquarters. Please submit promptly. Names received by May 15th will be placed in the Memorial Book for National Convention.

Important: If you have not already done so, please also submit an Empathy Concern form to your State Empathy Chair so she may respond to the deceased member's next of kin. For deaths occurring after May 15th, call National HQ at 1-800-678-0389.
