Report of Deceased Member
National HeadquartersUse this form to report the death of a chapter member to National Headquarters. Names received by May 15th will be placed in the Memorial Book for National Convention.
Important: Please also submit an Empathy Concern form to your State Empathy Chair. For deaths after May 15th, call National HQ at 1-800-678-0389.
Request for Transfer of Membership
Chapter PresidentComplete this form when a member of your chapter moves to a new area. The receiving chapter's membership chair will be notified to extend a Kappa welcome.
For Chapter Presidents. Submitted to National Headquarters. The receiving chapter's membership chair will be notified to welcome the transferring member.
Incentive Award Report
National HeadquartersReport a Kappa member who has sponsored three or more new or reinstating members during the 2025-2026 fiscal year. Dues must be current. Submitted by the Chapter President.
Deadline: May 1. Awards are issued upon receipt of this form together with each new member's address form and initiation fee at National Headquarters, 1875 E 15th St, Tulsa, OK 74104.
State Philanthropic Report
President-Elect & Philanthropic ChairDocument the donations of time, energy, and monetary gifts reported by your state's members during the current year. Submitted by the State Philanthropic Chair or State President.
Deadline: May 15. Submit to the National President-Elect and National Philanthropic Chair. Kempe Foundation checks should be made payable to "Kempe" and mailed separately by November 15 to National Headquarters.
Nominating Committee Delegate & Alternate
National HeadquartersCertify the elected Delegate and Alternate to the National Nominating Committee meeting, June 2026, Charlotte, NC. Submitted by the State President.
Deadline: May 1. Each member must have attended two of the last five National Conventions. No state may send the same member more than two consecutive years. A candidate for national office may not serve on this committee.
Request for Visit to State Chapter
National PresidentRequest a visit from the National President or their representative to your state chapter. The state must plan a prospective member function for the visit.
Deadline: September 1. The 2025-2026 visitation schedule includes: Florida, Georgia, Missouri, South Carolina, Texas, and Arkansas. States on the rotation have expenses paid by National. All other states must cover expenses including lodging.
The Kappa Profile — News Release
The Kappa ProfileSubmit news for publication in State and National newsletters including The Kappa Profile. Scholarship awards, community service projects, and special honors to members are of general interest.
Submission Guidelines: Include details of who, what, when, why, and how. Include the chapter and state of any members mentioned. All copy is subject to editors' approval and space availability.
